The new patient intake appointment is scheduled for 60 minutes; 15 minutes for administrative policies and procedures and 45 minutes for our first session together. After this our appointments will be 50-60 minutes long.
When we first meet we will talk about what brings you in, what your goals are, and formulate a working model of how we can work together to achieve those goals. This is a good time for you to ask questions about our work; asking questions is something I invite you to do throughout our relationship, as more will arise the longer we know each other. Ultimately, when you are working with a therapist with whom you feel safe and met, the relationship itself is healing too.
We will begin with administrative aspects of our relationship, such as session times and payment. Please bring the following items to our first appointment:
- Insurance card; I accept Blue Cross Blue Shield (BCBS) PPO
- Form of payment; cash, personal check, Zelle, credit card and HSA debit/credit cards are accepted.
- Please print this “New Patient Intake Form” (PDF to Print and Complete) and bring it completed to your appointment (or arrive 10 minutes early to complete this in person).
My initial new patient intake fee is currently set at $210 with individual therapy sessions afterwards billed at $185. In certain circumstances, I am also able to offer sliding scale sessions for clients.
I am registered as a PPO provider for Blue Cross Blue Shield (BCBS) and can provide a Superbill invoice for you to submit claims to any out-of-network insurance providers. I do not accept other insurance providers at this time.
When you are ready to set up an initial new patient intake session, please call 773-303-6477 to get scheduled. Please have your BCBS-PPO insurance information available when you call.
